2025-2026 Forms
Submit completed forms through your MyCardinalConnect student portal, under the Financial Aid tab. Some forms cannot be uploaded there, so you will have to submit them below. For your own security and to protect the privacy of your student account, the Financial Aid Office can no longer accept forms via email. All filled forms must have a real "wet" signature; electronically signed forms will not be accepted.

Dependency Change
If the Department of Education has determined you are a dependent student and you:
- did not report information about your parent(s) on your FAFSA
- and/or indicated that you may have a circumstance that requires you to follow up with your financial aid administrator
Please read the requirements of the Dependency Change Request form, and complete it if needed. The Financial Aid Office cannot determine your eligibility for financial aid until more information is provided. Please be advised that submitting this form does not guarantee changes to your financial aid awards.
Special Circumstances
If you or your family are experiencing extreme changes to your financial situation (i.e., loss of employment, medical expenses, separation/divorce), we may be able to adjust your FAFSA to reflect your current situation.
